Congratulations to CCMH on receiving the 2024 Excellence and Quality Award for Hip and Knee Total Joint Infection and Readmission Reduction from the Oklahoma Hospital Association. This achievement is a testament to the dedication and hard work of the Education, Risk and Quality Department, but also to Dianna Marin, coordinator for the Joint Program.
This project focuses on decreasing NHSN reportable hip and knee joint infections and decreasing the CMS complication rate for the joint program. The program includes pre-education prior to the patient’s surgery that lets them know what to expect with the procedure and also what to expect when they return home. A 24/7 nurse navigator reaches out to the patients the night before surgery, the day following surgery, and then several phone calls are made once the patient is discharged. This contact with the patient continues for the full year following the procedure.
